Your personal and health information is used to facilitate your enrolment, schedule your physician intake appointment, and deliver personalised protocol recommendations under physician oversight.

We use your contact information to send you programme updates, appointment reminders, and communications relevant to your care. You may opt out of non-essential communications at any time.

Aggregate, de-identified data may be used to improve programme quality, evaluate outcomes, and conduct internal research. This data cannot be used to identify you.

Mino Medical, Inc. is a covered entity under the Health Insurance Portability and Accountability Act (HIPAA). Your Protected Health Information (PHI) is handled in accordance with HIPAA's Privacy Rule and Security Rule.

Elation EMR: We use Elation Health's electronic medical record platform to manage your clinical records and facilitate appointment booking. Elation acts as a Business Associate under HIPAA and is bound by a Business Associate Agreement (BAA) with Mino.

Mino Medical Team: Your information is accessible to licensed physicians and clinical staff at Mino who are directly involved in your care. Access is limited to those with a clinical need.

PA-SBA: The Pennsylvania State Troopers Association and its affiliated bargaining units receive only aggregate, de-identified programme participation data for benefit administration purposes. Your individual health information is never shared with your employer or union.

We do not sell, rent, or trade your personal or health information to any third party for marketing purposes.

You have the right to access, correct, or request deletion of your personal information held by us, subject to applicable legal and clinical record-keeping requirements.

Under HIPAA, you have the right to request restrictions on how your PHI is used or disclosed, to receive an accounting of disclosures, and to request that communications be made through alternative means or at alternative locations.

California residents may have additional rights under the California Consumer Privacy Act (CCPA), including the right to know what personal information is collected and to request its deletion.
